This is a self-assessment of your confidence in the competency areas covered in this course. You will take the same survey at the end of the course to help us evaluate the effectiveness of the training.

For this last form you will complete the form below. Simply enter your answers in the spaces provided and click "submit." If you do this correctly, a copy of the form will be sent to the email address you enter on the form. This is also the way you will submit assignments for this course.
